Case Manager
Permanent Full-Time
Salary: LT-3 $78,632.35 – $110,263.74 per annum ($43.20 – 60.58/hr)
(depending on education and experience)
Overview
Under the direction of the Wellness & Education Director (WE Director) this position is responsible for the management of wellness and support programs, services, and staff, and is responsible for providing community based social services to identified groups, individuals and families in order to improve the social well-being and functioning of families and individuals.
Temporary assignments from other governments will be considered. Please ensure you have your supervisor’s approval prior to applying.
Duties
- Responsible for the provision of individual/group/family assessment, counselling, and treatment services, including aftercare support, educational sessions, and facilitation of community-based programming and community development services.
- Works directly with clients, assisting with the development and implementation of client wellness plans, including their involvement with other providers of services. Contributes to the development and implementation of Wellness Plans in conjunction with the client, family, KFN Wellness team and other support services, while establishing and maintaining relationship with client based on respect.
- Administer social assistance upon application within applicable legislation and KFN approved policies.
Qualifications
Degree or Diploma in Social Work or relevant degree and/or a minimum 5 years’ experience working within social programs and services. Experience working in a cross-cultural and team environment.
Conditions of Employment
Valid Class 5 Yukon Driver`s license and Criminal Records check.
